| The Palm Tree Center for Nursing & Rehabilitation accepts Medicare, Medicaid and Private Pay. Please be aware that Medicare only covers skilled nursing under certain provisions and your eligibility will be evaluated by the facility. If your stay is fully covered under Medicare, you will be entitled to 20 "full" days and 80 "part" days. The copay for the 80 part days is usually picked up by Medicaid (if eligible). The facility will ask for certain documents from you to determine Medicaid eligibility. Our Admissions Department will be happy to assist you in preparing the Medicaid application at no charge for you and your loved one. In a private pay case, the facility's rate is $275/day all inclusive. If you have any questions about skilled nursing and rehabilitation coverage please call our Admissions Department and we will glady answer all of your questions and concerns. |

Directions to the Facility: Belt Parkway- Take 14th avenue exit off the Belt Parkway. Make a right at the light. Make a left onto 15th avenue and end at 5606 15th avenue. Train- M & D Lines to 55th street Bus- B16 (Fort Hamiliton Pkway Bus) to 14th ave and 56th street. |
